Clinical Triage and Rehab Support Practitioner - Criteria
·Registered Nurse, Physiotherapist, or Occupational Therapist
·Current NMC or HCPC registration
·5+ years post-qualification experience
·Experience with neurologically impaired adults
Clinical Triage and Rehab Support Practitioner - Duties
·Conduct triage assessments for patients with neurological and other complex conditions
·Work closely with the multi-disciplinary team to determine patient suitability for rehabilitation
·Provide clinical reasoning and recommendations for rehabilitation pathways
·Line manage the admissions and discharge coordinator
·Support ongoing development of clinical protocols and processes
·Liaise with acute services and GPs to ensure safe patient admissions
