Salary: £36,000 – £46,000 (pro rata depending on experience)
Hours: 30 hours part-time (37.5 hours full-time)
We are looking for an experienced Clinical Triage Practitioner with a strong background in neurorehabilitation or working with patients presenting with neurological diagnoses. The ideal candidate will have at least five years of hands-on experience in this specialized field, ideally within a rehabilitation setting. You will play a crucial role in assessing, identifying impairments, and making tailored rehabilitation recommendations for patients with complex needs.
Clinical Triage Practitioner - Key Responsibilities:
- Provide autonomous clinical triage, including assessments for patients with neurological conditions or complex presentations.
- Use specialist clinical reasoning and evidence-based practices to assess, diagnose, and make recommendations on appropriate care pathways.
- Manage triage calls effectively, using excellent communication skills to ensure a professional and compassionate experience.
- Evaluate patient progress, write reports, and ensure smooth handovers to the appropriate care teams.
- Work closely with acute services, GPs, and referrers to ensure the best clinical outcomes for patients.
- Contribute to the development of trusted assessor processes.
- A professional with at least four years of experience in neurorehabilitation or neurological conditions.
- NMC or HCPC registration
- Experience with triage
- Strong clinical reasoning and problem-solving skills.
- Ability to work autonomously and collaborate with a multidisciplinary team.
- Excellent communication and interpersonal skills to engage with patients, families, and healthcare professionals.
- Knowledge of professional guidelines and a commitment to high standards of patient care.
