Care Management Manager – Registered Nurse
South Central Westchester | Full-Time | Monday–Friday | Day Shifts + On-Call
Salary: $125,359 – $188,049 DOE

We are seeking a dynamic and experienced Care Management Manager (RN) to oversee the delivery of high-quality care coordination within a respected healthcare organisation in South Central Westchester. This is a leadership opportunity ideal for an RN with a strong background in case management and a passion for driving performance and patient outcomes.

Key Responsibilities:
  • Lead and manage the daily operations of the Care Management department.
  • Support staff development, performance, education, and competency assessments.
  • Coordinate with Utilisation Management to ensure patient safety and discharge planning efficiency.
  • Oversee department quality assurance, compliance, and performance improvement initiatives.
  • Build and maintain strong relationships with community agencies and external partners.
  • Ensure adherence to Medicare, Medicaid, HMO, and other regulatory standards.
About You:
  • Registered Nurse with a Bachelor’s degree in Nursing (Master’s preferred).
  • 5+ years’ managerial experience in an acute hospital setting.
  • Prior experience as a Case Manager required.
  • Strong working knowledge of regulatory requirements, insurance processes, and care coordination.
  • Proficiency in Milliman/InterQual criteria.
  • PRI/Screen and/or CCM certification preferred.
  • Excellent communication, leadership, and analytical skills.
