Mental Health Nurse – Home-Based Care in The Bronx

Full-Time | Monday–Friday (9 AM–5 PM)

Salary: $85,000–$90,000 Annually | 35 Hours per Week

Role Overview:

As a Home Care Mental Health Nurse, you will play a pivotal role in assessing and addressing the mental and physical health needs of clients. Working closely with a multidisciplinary team, you will provide medication management, health education, and therapeutic interventions while coordinating care with community providers in The Bronx.

Key Responsibilities:

• Conduct comprehensive assessments and develop personalised care plans tailored to individual mental health needs.
• Collaborate with a multidisciplinary team to deliver evidence-based interventions and treatments.
• Administer and monitor psychiatric medications, ensuring adherence and addressing any concerns or side effects.
• Educate clients and their families on mental health, treatment options, and wellness strategies.
• Participate in crisis intervention and de-escalation efforts to ensure client safety.
• Coordinate with external resources, such as primary care providers, housing agencies, and social services, to enhance the continuity of care.
• Maintain accurate documentation of interactions, treatment plans, and progress notes.
• Contribute actively to team meetings, case discussions, and supervision sessions.

What We’re Looking For:

• A licensed and experienced Registered Nurse with a focus on mental health care.
• Strong assessment, medication management, and crisis intervention skills.
• Based in or around The Bronx
• Exceptional communication and collaboration abilities to work effectively within a multidisciplinary team.
