Medical Social Worker (SW Care Manager)
South Central Westchester | Full-Time | Mon–Fri 9am–5pm + Weekend Rotation
Salary: $87,658 – $131,487 DOE

A respected healthcare provider in South Central Westchester is looking for a compassionate and skilled Medical Social Worker to join their Care Management team. This is a full-time role supporting patients across all populations to ensure safe and effective discharge planning, reduce readmissions, and coordinate post-acute care services.
Role Overview: You will conduct comprehensive assessments, identify discharge planning needs, and support transitional care from admission through discharge. You’ll collaborate with multidisciplinary teams to ensure patients receive the appropriate aftercare support and services, in line with hospital goals around length of stay, readmissions, and patient satisfaction.

Key Responsibilities:
  • Complete timely psychosocial assessments and reassessments throughout hospital stay.
  • Coordinate aftercare services, including patient/caregiver education, insurance coordination, and high-utiliser support.
  • Collaborate with the multidisciplinary team and communicate discharge needs across services.
  • Address complex social, emotional, and financial barriers to care.
  • Participate in quality improvement and department initiatives.
  • Maintain up-to-date and accurate documentation in line with departmental policy.
About You:
  • Master’s Degree in Social Work from an accredited institution.
  • Licensed Social Worker (LMSW or LCSW) in New York State.
  • 2+ years’ experience in an acute care or related setting preferred.
  • NYS SCREEN Certification (or willingness to obtain within 3 months of hire).
  • Strong interpersonal and assessment skills, with the ability to manage complex patient situations.
  • Comfortable working independently and collaboratively in a high-pressure environment.
